Registration and Scheduling
The admissions staff is here to help throughout the registration process. In order to serve you better, we will need the following information each time you register for either an inpatient or outpatient visit:
- Patient name, address and contact information
- Patient social security number
- Patient religious affiliation (if applicable) and marital status
- Patient employer
- Patient insurance information
- Physician's name
Verifying patient information is important to avoid unnecessary delays.
Scheduling
Our facilities offer a variety of outpatient services. Many of these services require a scheduled appointment. You or your physician may schedule these appointments.
Call the hospital at (570) 770-3000, Monday through Friday, 8:30 a.m. - 5 p.m.
In order to serve you better, scheduling staff will need the following information each time you call to schedule a service:
- Patient name, address and contact information
- Patient social security number
- Service required
- Reason for service
- Patient insurance information
- Name of the physician ordering the service
